Meaning of "Real of fact"

One patient said on telephone to me that “ he has taken medicines for pain but this had no real of fact”.

What he wants to say or i heard something else than correct wording?

I'd say that what you heard as "of fact" was actually the word effect:

He has taken various medicines for the pain but this has had no real effect.
