This is about the disadvantage of HRT (hormone replacement therapy) skin patches:

"May cause breast tenderness, spotting, and bloating."

What does spotting mean in this context?

'Spotting' here means that spots (usually round coloured marks) may appear on the breasts.

Are you sure about that meaning? To me, 'spotting' in such a context would refer to very slight (and probably irregular) vaginal bleeding.

I only just saw your post, Amy. This other meaning of ‘spotting’ also crossed my mind and is probably the right one in the context. However, I somehow associated the three symptoms listed to the breasts.
